Forms & Required Documents

Please ensure all required forms are completed and signed by the assigned due dates.

A current physical must be completed and on file before a student is permitted to practice or participate in any Royal Hive activities.

Required forms include:

• Parent Contract (must be read and signed)

• Royal Hive Contract (student agreement)

• Royal Hive Handbook Acknowledgment

All families are expected to read the handbook in full before signing. A physical copy of the Royal Hive Handbook will be provided to each team member.

Failure to submit required forms by the due date may result in removal from the team
